For new contact lens wearers at BayCare Clinic/Green Bay Eye Clinic, fits include the following:

  • Contact lens evaluation
  • Selection of contact lenses
  • Solution starter kit
  • Contact lens delivery visit 
  • Appropriate parameter changes if needed

Contact Lens Evaluations

Green Bay Eye Clinic contact lens wearers need to be seen for yearly contact lens evaluations as well as a complete eye exam every one to two years as recommended by their doctor. Extended wear and disposable contact lens wearers need to be seen more frequently.

The cost of the evaluation, all contact lenses, and contact lens follow-up visits are separate charges from the medical eye exam with your doctor.

Office Visits

Those patients who are fit with contact lenses and who have been charged a fitting fee, will have follow-up visits for the duration of the warranty period (45 days) at no additional charge.

Those patients who order replacement contact lenses will get one follow-up visit at no charge to check the new contact lens; however, any evaluation or other office visit beyond the original fitting date will be assessed a charge.

Insurance and Billing

Contact lenses and evaluations must be paid in full at the time of service, most insurance companies do not cover these expenses.  Insurance information will be collected on the day of your evaluation.  By request, exams, office visits and contact lenses will be billed to your insurance.  When payment is received from your insurance company, you will be reimbursed. 

Refund Policy

Green Bay Eye Clinic has a refund policy on contact lenses. When you are fit for contact lenses, you are assessed a fitting fee and a material (contact lens) fee. If you are not satisfied with your contact lenses, we will refund the material fee within the first 45 days. Professional fees are not refundable. There are no refunds on replacement contact lenses.

Contact Lens Prescription Release

If contact lenses have been fit by Green Bay Eye Clinic and the patient has returned for a contact lens check to verify the lens fit, the contact lens prescription can be released to the patient or to a licensed practitioner responsible for follow-up care.

The contact lens prescription is valid for one year from the final lens fitting.
